Service Descriptions

1. Online Doctor Consultation: Remote consultations with a doctor provide medical advice, follow-up care, and prescription services without the client needing to leave their home.

2. Nursing Supervision: Professional nurses will monitor the care worker’s performance and the client’s health, management of medications, and provide supervisory nursing care. This helps ensure that the care provided is safe, effective, and aligns with established health standards, ultimately enhancing the overall quality of care for clients.

3. Social Work Support: Social workers provide emotional support, offer counselling, and help the client navigate social services. This service helps improve the overall quality of life by ensuring the client remains mentally and emotionally healthy.

4.  Home Care Worker: This option provides continuous or partial care throughout the day or night, assisting with personal care, and daily activities, and ensuring the client’s safety, based on the level of supervision needed. Experienced senior care workers will monitor the client’s health, manage medications, and provide necessary nursing care. This helps in preventing health complications and managing chronic conditions effectively.

5.   Hospital Assistance: When a client needs to visit the hospital, the caregiver supports them emotionally and helps manage communication with doctors, ensuring they receive the best care.

6. Outdoor Companionship: This service helps elderly clients engage with the community, attend social functions, and run personal errands with the help of a trained caregiver.

Preventive Services  (Only for those who are NOT in Assisted Living)

·  Online Doctor Consultation: Clients will have access to online consultations with doctors for any medical concerns. This service provides convenient healthcare solutions, allowing them to receive medical advice without leaving home.

·  Nursing Supervision: Professional nurses will visit the client’s home every month to provide health check-ups and ensure personal care. This service helps to prevent from health related problems of elderly.

·  Social Work Support: Social workers will visit every month. This service enhances the mental well-being of the client and ensures they stay connected with a healthy society.

Personalised Home Care Services   (Those who are in Assisted Living)

These services can be customized and added to the basic package to meet the client’s specific needs. They include options for full-time care, night care, hospital assistance, and outdoor companionship.

·   Home Care Worker – Day & Night: This service provides round-the-clock care to assist the client with daily tasks such as medication, hygiene, diet, and mobility support. It is ideal for clients who require constant supervision due to health conditions or mobility challenges.

·   Home Care Worker – Day Only: A caregiver will assist during the daytime, helping with personal care, and daily tasks such as medication, hygiene, diet, and mobility support. This option is perfect for clients who need daytime support but can manage on their own at night.

·   Home Care Worker – Part-Time (Task-Based): This service is designed for clients who need specific assistance for only part of the day (a service – up to 3 hours). The caregiver will help with key tasks such as personal care, diet, and companionship.

Supervisory Services   (Those who with Home Care Services)

The following services are essential and included in every home care plan, offering foundational care and support:

·  Online Doctor Consultation: Clients will have access to online consultations with doctors for any medical concerns. This service provides convenient healthcare solutions, especially for clients with mobility issues, allowing them to receive medical advice without leaving home.

· Nursing Supervision: Professional nurses will monitor the care worker daily by phone and visit the client’s home four times a month for supportive supervision to care worker and to provide health check-ups, manage medications, and ensure personal care. This service helps to maintain the health of elderly clients and monitor any changes in their condition.

·  Social Work Support: Social workers will visit four times a month, offering emotional support, connecting the client with necessary community resources, and counselling the family. This service enhances the mental well-being of the client and ensures they stay connected with society.
