Service Descriptions

1. Online Doctor Consultation

Remote consultations with qualified doctors provide clients with medical advice, follow-up care, and prescription services—all without leaving home. This ensures timely medical support and convenience, especially for clients with mobility challenges.

2. Nursing Supervision

Professional nurses oversee both the care worker’s performance and the client’s health status. They manage medications, provide supervisory nursing care, and ensure all services meet professional health standards. This guarantees safe, effective, and high-quality care.

3. Social Work Support

Trained social workers provide counselling, emotional support, and guidance in accessing social services. This helps maintain mental well-being, promotes social connectedness, and enhances the client’s overall quality of life.

4. Home Care Worker

Care workers provide continuous or part-time support based on the client’s needs—day, night, or task-based. Services include assistance with personal care, daily activities, medication management, and safety monitoring. Experienced caregivers also help manage chronic conditions and prevent complications.

5. Hospital Assistance

When hospital visits are necessary, caregivers accompany clients, offer emotional support, and facilitate communication with doctors. This ensures clients receive clear, coordinated, and compassionate care.

6. Outdoor Companionship

Clients receive support from trained caregivers to attend social functions, community events, or personal errands. This promotes social engagement, independence, and a better quality of life.

Preventive Services

(For clients NOT in Assisted Living)

  • Online Doctor Consultation – Access to remote consultations with doctors for timely medical advice and preventive care.
  • Nursing Supervision – Monthly home visits by professional nurses for health check-ups and personal care support, helping to prevent health-related issues.
  • Social Work Support – Monthly visits by social workers to provide counselling, emotional support, and social engagement opportunities.

Personalised Home Care Services

(For clients in Assisted Living)

These services can be tailored to each client’s needs and added to the basic package:

  • Home Care Worker – Day & Night: Round-the-clock support with personal care, medication, hygiene, diet, and mobility. Ideal for clients needing continuous supervision.
  • Home Care Worker – Day Only: Daytime assistance with personal care, daily tasks, and health management, suitable for those independent at night.
  • Home Care Worker – Part-Time (Task-Based): Flexible support (up to 3 hours/day) for specific tasks such as diet management, hygiene, or companionship.

Supervisory Services

(For clients with Home Care Services)

These essential services are included in every home care plan to ensure comprehensive support:

  • Online Doctor Consultation – Convenient access to doctors for medical concerns, especially helpful for clients with limited mobility.
  • Nursing Supervision – Nurses provide daily monitoring of care workers (via phone) and visit the home four times a month for check-ups, medication management, and supportive supervision.
  • Social Work Support – Social workers visit four times a month to provide counselling, connect clients with community resources, and support families in caregiving.
